Job Description & Responsibilities:
JOB SUMMARY In the home health care setting, Licensed Practical Nurses (LPN) provides one-on-one skilled nursing care and treatment to patients in their home. Assesses patient needs and develops a plan of care with the physician. Carries out the care plan with patient, family, and support staff other support staff. nurse assistants. Monitors and evaluates patient response to treatment, medication, and healing or mobilization progress. Oversees case management. Use a comprehensive and patient-specific care plan with innovative solutions. Spend quality time with the patient, in their own home, to promote their health and independence.
Qualifications
EDUCATION REQUIREMENTS Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing. (Required) EXPERIENCE REQUIREMENTS 1 - 2 years Practical Nursing Experience (Preferred) CERTIFICATIONS AND LICENSURES Required Certifications/Licensures: Licensed Practical Nurse (LPN) with current Georgia license, Basic Life Support (BLS) Preferred Certifications/Licensures: Advanced Cardiac Life Support (ACLS) Must possess and maintain a valid Georgia Driver's License.
