Job Description & Responsibilities:

Determine and approve eligibility for applicants to receive services on a weekly basis. Enroll eligible applicants by completing the FSOP and sending it to the region for verification. Enter client information into the ETO system. Complete Service Summary Plans on each new client upon approval and annually during birth month using ETO system. Determine appropriate use of funds per client requests and budget allowances. Maintain knowledge of clients and their needs. Coordinate and arrange services and purchases for clients. Approve time sheets and invoices for payment weekly and enter into ETO system and WIS. Document purchases, conversations, situations, etc. as needed using ETO system. Explain program guidelines, limitations and requirements to applicants and clients. Ensure client satisfaction by assisting all in timely and considerate fashions. Receive and return phone calls, greet and assist clients, families and others desiring program information daily. Distribute correspondences, newsletters, updates and other information to families as needed. Develop knowledge of available resources for clients. Inform families about the Medicaid NOW/Comp waivers and assist with initial applications when necessary. Insure all clients receive support that is in compliance with all applicable standards and regulations. Insure all client rights are appropriately respected and honored as well as report any violations. Maintain current client records on-site. Insure submission of required documents to various departments as scheduled. Utilize community and any other resources available to insure all client needs are met. Conduct annual client satisfaction surveys using ETO system. Review Client satisfaction surveys & implement changes when feasible. Prepare reports, collect information and participate in the management and planning of Family Support funds. Implement new program guidelines and agency procedures. Maintain knowledge of program guidelines. Discharge clients from Family Support services when appropriate. Promote awareness of other Easter Seals Southern Georgia services which may be applicable such as Respite, Residential, Vocational or Day Services. Notify Director of Respite & Family Support Services of questionable situations requiring second opinion. Maintain cleanliness and functional operation of local satellite office. Complete continuing education as required for growth and development. Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure client protected health information is secure. Maintain knowledge and understanding of Easter Seals Southern Georgia policies and procedures. In the absence of the Director of Respite & Family Support, may assume duties and responsibilities of this position including interaction with Regional Boards, Clients, Families and ESSG staff as well as other leadership responsibilities as appropriate. All other duties as assigned.

Qualifications:

Minimum Qualifications: High School Diploma or GED required. Preferred Qualifications: Bachelor’s Degree in related field of study preferred. Two or more years of work experience in a related field.

Salary:

$19.00 - $20.00 Hourly
