Position Summary: The Care Advisor conducts intake and provides proactive customer service for individuals who may be eligible for Careforth services. This includes inbound and outbound communication with prospective care recipients, caregivers, and professional referral sources. This position works as part of a centralized intake and support team to qualify and enable enrollment into Careforth programs across multiple states. Serving as the organization’s “front door”, this individual shares information about Careforth services, gathers eligibility information and routes individuals to the appropriate stop along the enrollment process. This involves gathering and documenting eligibility information including demographic, clinical and financial qualification criteria. The Care Advisor will provide guidance to caregivers, ensuring excellent customer service and advocacy. #LIREMOTE What You Will Do: Perform intake and outreach to engage with potential caregivers, care recipients, and referrals sources, providing information on Careforth services specific to their geography Provide proactive outreach and follow-up to expeditiously advance qualified individuals through the eligibility and enrollment process Provide excellent customer service using empathy, inquiry and advocacy Gather and document required eligibility and credentialing data utilizing multiple systems Maintain accuracy, timeliness and confidentiality to ensure required service levels and quality standards are met, including HIPAA requirements Proactively communicate and collaborative with other departments including Sales, Enrollment and Care Management teams Guide callers who do not meet eligibility requirements to other professional and governmental resources Participate in special projects and performs other duties as assigned
